What is Home Health Care?
Home Health care are services that are medically necessary and ordered by a physician. These are services that enable an individual to receive care at home and decrease the possibility of facility placement. Home Health services also assist in recovery at home after major hospitalization. Home care is best suited for those that do not need 24 hour supervision or monitoring. The program of care can be short or long term.
We offer complete health care services through a comprehensive team effort by patients, families, nurses, therapists and physicians. As medical needs change in your life, let us create a plan of care for you.

Referrals
Referrals most often come from the physician, hospital or nursing facility, but patients and families can seek these services on their own as well.

Home health care services may be covered by:

- Medical Assistance
- Private Insurance
- Veterans Administration
- Waivers
- If not covered, patients may pay privately or be eligible for a reduced rate on our sliding fee scale based on ability to pay.
